Scholarship & Player Forms
Type Name Description
PDF NWN Medical Release
 This form should be used by all Guest Players.
PDF NWN Cash/Corporate Donation Form
 The Sponsorship form is for all donations made to the Club.
PDF NWN Scholarship Application This form explains how to apply for an NWN scholarship based on financial need.
PDF NWN Scholarship Guidelines This form contains information on the school lunch rate and how best to qualify for one of the NWN scholarships based on financial need
PDF Guest Player Use this form to allow a NWN player to participate as a guest player on another team for non-league or non-cup play with the permission of the player’s coach. This form may not be needed from NWN team to NWN team with the advent of the Club Pass (soon to be adopted by WSYSA).
PDF Request to Travel within the US This form describes the documents that each team must carry when traveling to another state.
PDF Request to Travel outside the US This form describes the documents that each team must carry when traveling outside the US for competitionst internationally (including Canada).
PDF Player Status Form Use this form as the WSYSA status form for non Sam users
PDF WSYSA Medical Accident Insurance Claim Form This form must be completed in order for a claim to be reported to the WSYSA.

PDF Player Registration Form Form to be completed and signed by all parents for players in this club
